The Hanky Code is a system used mainly by gay men, but also by lesbians, to signal what they're up for, by placing a handkerchief or scarf of a certain color usually hanging out of their left or right back pocket. Left side means one is desiring to be active or top, and right side means one wants to be recipient or bottom. Like black on the left means one would like to adminster (heavy) S&M. Right side means one would like to be the bottom in an S&M scene. Grey would mean bondage. Yellow on the left means one would like to be the giver of golden showers, on the right it means one would like to be the recipient. Red means fisting, so the guy on the picture would like some.
Some of the lists of color codes makes one wonder how many of them are made up, as there seems to be altogether too many nuances and meanings to keep track of. But here's a good list, which outlines which ones are common, less common, and rare.
